I feel like it did unfairly bear the burden of the whole "dogfighting is obsolete" thing. I would also imagine the transition to the F-15, 16, and 18 probably had a lot to do with electronics and computing advancements that the F-4 was never designed to capitalize on. That's largely conjecture though.

None really - it's just a decision they made to get the engine placement they wanted.

Yes, there was a need at the time for an improved fighter and improved fighter bomber to use against the Soviet Frontal Aviation forces. The F-4s were still going to be used as bomb trucks but were expected to suffer heavy losses in any likely large conflict. Thus the F-15 was intended for air superiority, and the F-16 as a air-combat partner that also was good at strike roles.

They didn't really have that many low-time airframes. Much of the fleet had been flown pretty hard by the early 80s. The Israelis and I think the Turks both did major upgrades like you mention. No idea how much maintenance the improved versions need.

Keeping the tail high helps mitigate tailstrike issues. Keeping the engines and nozzles forwards near the CG lets you employ big-ass heavy engines without introducing major weight and balance issues.
