Radiation protection tech here. Time, distance, shielding. It would only be an effective area denial weapon over a small area. The further you are from it, the less dose you take. Put some shielding up(like the corner they hid behind) and you take less dose. Besides that, it's extremely heavy, and has a high likelihood of giving everyone involved with it dose.
Basically, the guys storing/loading/deploying it would have more ill effects than any enemy you tried to use it against.

The picture is still incorrect though. And I doubt looking at it would instantly kill anyone, I don't think anyone died instantly from radiation anywhere at Chernobyl apart from the blast of the initial explosion.
The half lifes of those materials lasts hundreds to thousands of years, it will still be incredibly radioactive, but this picture is all about impressing 12 year olds and retards on the internet.

Watch some of the documentaries on Chernobyl. After the accident a team of scientists made multiple trips inside to track the nuclear material down, they found the elephants foot. They mostly died of cancer but it was years later.
