It's likely just a fairy. Perhaps one related to the poison flowers. Why it's there is unknown but it's not unusual for other characters to have decorations. Medicine probably isn't a Tsukumogami either. Creepy living dolls are basically their own thing, even in the west.

Doing a bit more personal research on the concept of Tsukumogami, it's abundantly clear that Medicine is meant to represent one, from her origin to much of the dialogue directed at her, especially from Eiki.

She's also pretty clearly meant to represent one. She never really leaves her hill until PoFV (and even then it takes her several stages to do so) and she closes herself off to others. Her lecture from Eiki directly references shut-ins, and could be seen as a criticism of that type of lifestyle. Hell, Sakuya literally calls her a shut-in ingame. There's very little room for debate here.

Not the guy you're responding to, but by definition it is impossible for Medicine to be a shut-in, because that term specifically refers to someone who confines themselves indoors, and there's no indication that she lives anywhere other than outside on the hill. Flandre would be an example of a shut-in, while Medicine was just a recluse.
