The only reason she gets lumped in sometimes is because of the -douji suffix of Konngara-douji, but it just means child.

Konnagara-douji himself doesn't have anything with oni, but is a follower of Fudo Myo, and is sometimes mentioned as a yaksha. There's a possibility yaksha served as the basis for oni, but that's as far as the connection goes.

Also it looks like a spike through her head to some, not a horn.

If you believe that the PC98 events happened in the past (with Reimu's mother or something like that) then Konngara could possibly be the 4th.
Since Reimu detoured into Hell in order to fight her, and the Oni were moving down into hell within the last 60 years, I think?
