Zun will sell Touhou to the highest bidder as soon as the cirrhosis kicks in so he can support his family.

If he dies in an accident or something pretty much ebery big company in Japan will go and try to buy the rights to his family or whatever small firm gets the rights after he dies..

Afterwards you can expect a Touhou anime, overexplotation of the IP, and pretty much the death of whatever integrity the franchise has left at that point.
