I was like that, but through using the "save link in folder" firefox extension to pre-sort my pictures and doing a big manual sort once every year I manage to keep them neat enough.
Then again I only have a total of around 11000 pictures, which is small compared to some collections I have seen.

It's a fucking mess, honestly. Most often it goes in my /a/ folder for being japanesey, but sometimes I'll put touhou stuff in the /v/ folder because it's a video games. then I keep my lewds and wallpapers each in separate folders... I should be shot.

I just save my images to the Images folder on my pc.

I use a local booru program and tag everything with my own set of tags that makes them easy to find.
