Kosushit did not realize Aya was Aya, let alone even a crow tengu, until Aya took off her reporter hat and put on her crow tengu hat. This despite the fact that Aya has visible elf ears.

It seems apparent then that a large amount of a Touhou's essence is contained in their individual ZUN hats. Touhou's can survive without them, but they lose enough of their essence that humans at least can't distinguish them from other humans.
