Yes. But english is a good start if you want to live in Berlin. People in Berlin speak english a lot, but people in Munich are even not speaking german at all

It's not that simple here. We don't have USMLE or something similar. You have to do a language test first and foremost and they would evaluate if your degree meets the Norwegian requirements. You may end up having to do courses and extra practice. It's is easier afaik if you're already a specialist.

Don't know about Pakis, but Germany employs our doctors even if they don't know the language, the demand is too high, but they pay you German lessons, you're expected to become fluent in German eventually. We're EU members, though, so no degree nostrification is required.
