Maybe it's because my own dialect(let's say north Sicilian) is somewhat similar, but I never found Neapolitan particularly hard to understand. If you think it's difficult just wait until you hear Pugliese or even Calabrian dialects, to say nothing of Sardinian which is recognized by even the government as a separate language altogether.

It's hard to explain, but in short that is true. Dialects are grammatically similar to Italian but use extremely different words and sounds. Most of the population speaks Italian, albeit spoiled by common mistakes which vary widely in relation the geographical position. It is still possible to find people, usually in small and isolated communities, who can't speak Italian. Some regions have true languages with grammatical rules and a relevant literature. We understand each other however, at least most of the times.
