I'm not that based paki guy, but look up "hijras". They're the "3rd gender", almost always m2f transgenders. They have an odd culture but they're mostly accepted, some court case won them their rights a while back. They have a long history in South Asia. Every one knows they exist, it's not all underground.
There still are problems obviously, Pakistan isn't some tolerant western country, but IMO trans people are accepted here more than gays are.
