Control the Yucatan peninsula, Cuba, and take over Florida. You have denied the US a good chunck of its coast. It's one of the best launching points to launch an invasion into the US. I don't think you know what 'suburbia' entails for most of the country outside the coasts. It's literally just neighborhoods surrounded on all sides by wilderness. This process will obviously take years.
Helicopters can be shipped like everything else.
Drones will fit for the job too, perhaps better. In fact killing insurgents is a process that can be completely automated with current technology, drones are cheap and it would just a few years to kill everyone but a few lucky ones.

Ballistic shields are in fact very important, but they won't do much in case of a saturation attack, and in OP's scenario that would be the case.

A US-China alliance perhaps could survive but the numbers are still against them.

The US probably already has pretty much automated target tracking, aiming and firing systems for their drones, but they have someone to push the button and they don't massacre everyone for political reasons. I don't think human rights concerns them very much.
