As a German who's been to the US, I can confirm they do indeed have real bread. It's quite difficult to come by, though, and people pretend it's the second coming of Christ when it's actually (by German standards) just normal bread.

We have butchers, fisheries, bakeries and a café in almost every single little town. They're about as common as pharmacies.

We have those too, and they do take over butchers/fisheries in city centres, but bakeries and cafés survive, because you don't want to go into a city centre and park you car and walk half a mile inside it just to get a coffee or some bread. Those are things you usually go to by foot from home before/after work.

Small villages simply don't have the large super-stores if they are bordering a large city that does, so that's where they would/do strive: suburbs.

Well usually people skip it to make a uniform sandwich. By the time they've gotten to the end of the loaf the first skipped slice is gross and abused.

A few years ago I realized how stupid it is to skip it, so I just instantly started toasting it, eating it with jam, butter, a little brown sugar, cinnamon, cream cheese, or i'll use it as the first piece of french toast.
