Usually if someone is wearing shoes in your house its a guest whos just stopping by to bullshit and you didn't say anything about them wearing shoes on your carpet. It differs from family to family.

Not all the time, there's a bit of cultural nuance. you follow the the lead of the host and remove your shoes at the door if they're removing theirs. If you're not sure and they don't tell you to remove the when you come in I always ask if I should remove my shoes. If they have clean carpeted floors 90% of the tie I remove my shoes.
