You could think of "sandwich" as being kind of like an adjective meaning "between two pieces of bread". So a tuna sandwich is "tuna between two pieces of bread," a roast beef sandwich is "roast beef between two pieces of bread," and an idiot sandwich is "idiot between two pieces of bread."
I mean, sandwich isn't actually an adjective - the ingredient is the adjective - but you get what I'm saying.
