You have to have enough capital to do it. Shit is expensive.
You also need a physician to act as your medical director. The MD's license is what the medics actually provide care through.

We currently do not have a profession called Paramedics in government service, unfortunately. Currently the job is done by nurses and MAs who ride the ambulance (each government hospital and polyclinic in Malaysia has their own fleet of ambulances). We also have NGOs such as St Johns Ambulance and the Red Crescent offering ambulance and paramedic service. There's also private ambulances that you can charter.
