It's not photorealistic, and it was painted, pretty much all of it, although some elements are copied several times, and maybe there are photo parts way in the background or in textures and such (this is called matte-painting, and it allows you to make actually photorealistic art).

There def some photograph underneath it, you can see it clearly peaking out in the background, the trees especially. But the bulk of it is painted over. You can check out Tyler Edlins tutorials, he does pretty similar stuff and paints over photographs in some of them.
