Artist has always meant, purely, a person who creates. Visual, hand made arts are not the only type of art. You would, like other people have suggested, be looking for more specific terms like illustrator, draftsman, painter, etc. If you want a word meaning "person who makes art with a markmaking stick" than you need to make up a word for that.
