I use Manga studio/Clip Paint, real good.
Krita if you're broke but it's pretty slow with big brushes.

Cintiq, i've owned the Surface pro 1 and I currently own the Surface pro 4. i also own a cintiq 13. I would pick the cintiq with a desktop anyday over a surface.

I have a pro 3, so don't know how well the older versions handle it, but clip paint responds really well between the pen use and the screen touching.
