Under what circumstances can you not carry a normal pen with you? Pilot makes a line of fountain pens and felt-tip brush pens called the Petit, but I just checked and they're just a little larger than an Altoids tin. Pilot and Zebra both make really small ballpoint pens, but once you get that small you're probably better off just cutting a Hi-Tec-C refill down to size or something.

What's a good lead holder? I've been using mechanical pencils to avoid sharpening, so lead holders seem like a happy medium to get more line variation and whatnot. Thanks in advance.

Staedtler Mars Technicos are ubiquitous and high quality, but anything you find comfortable is OK. Make sure to get a rotary lead pointer. Leadholder.com used to have a fuckton of info about them but it's dead now. Here's a backup detailing everything you'd want to know:
