Why does everyone pretend that when a nuclear war starts every country nukes every other country.

Any of the US/Russia/China trio annihilating each other's infrastructure would (on top of good billion deaths just form nukes) utterly fuck the rest of the world economically and resource-wise, resulting in hundreds of millions more dead in short order.
Nuclear winter would just finish the job.
