Were the Muslims ever attacked during their 5 prayers or did they just not pray during the wars and Crusades? It seems like attacking them would've been the smartest thing to do if they were praying.

Islam has a very useful clause that states that you can ignore ANY of the rules in the Koran if you do so for the purpose of destroying or misleading the kuffir. So it's perfectly legal for a Muslim to eat bacon and drink wine, if he's doing so in order to trick a non-believer for the purposes of later betraying him, and equally legal for them to forgo prayers when on military campaign.

Nope. You can't deny Allah. That's one rule you can't defy to trick an unbeliever even under torture.
However, as others have said, to OP, you are allowed to disregard prayer of you are on a jihad. And during the early days when Mohammad was around they would lead prayer and half would pray and watch the backs of the other half and alternate when not actually engaged in battle. So, that's how they got around it. 5 a day isn't a hard rule where you are in the middle of a battle and have to stop and pray right in the middle of fighting.
