Not very much, the real Commodus was not a snivelling sissy but he still was power-hungry and insane. One part they actually do get right was where Commodus fought as a gladiator, though in real life all the fights were rigged in his favor.

As far as movies go relatively accurate in the personality (the series of events didnt happen like that of course)

Commodus was a skilled fighter but made Nero look humble and would kill people who were perceived better than him.
