The USA has over 300 cities with 100k+ populations. A lot of these warheads aren't nearly as large as the public imagination gives them credit for. Take the B83 ( most powerful nuclear free-fall weapon in the United States arsenal.), It would barely take out Manhattan. You'd need a dozen or so to literally reduce the island to ash, and then you've still got the rest (more sparsely populated bit of New York). By the time you've eliminated one major city, you've exhausted half your arsenal - and this is assuming they all are afforded the opportunity to detonate at the optimum location (they won't be).

no but some weapons will invariably be defective, the most important strategic targets will require multiple warheads also nuclear strategic thinking accounted for having enough weapons to conceivably eliminate a large portion of the opposing power's nuclear retaliation capabilities to 'win' a nuclear exchange which means destroying hardened silos in the middle of nowhere which might require 5 or more warheads to knock out each individual one. There's also hundreds of these silos.
nuclear weapons used to be inaccurate as hell and a lot of them were dropped from bombers. they also have expiration dates. As the nuclear material decays and you get too many other elements in your bombs. Which can affect yield or cause it to fizz.
Nuclear arms reduction treaties allowed the USSR and USA to look good on paper. Yet all they really did was take apart old nuclear weapons and replace them with new more effective nuclear weapons.
