The Romans usually pumped in water from outside their cities via the aqueduct system, which was badass as fuck. Those niggers would send specialists out to find the best water they could, which they would test by boiling it off to see how much residue was left behind. More residue meant more minerals and shit in the water, which isn't what you want. Less residue, the cleaner and clearer your water. They would take the best water from the mountains, and pump it into the cities to be used for drinking and cooking and shit, and they would pump in the worst water for their sewage system. They got the pressure they needed from raised aqueducts and cisterns that fed the system via gravity.

The Persians did the same thing, and actually better, as those niggers dug tunnel systems to get their water to their cities, and could even use it as a form of air conditioning.

The closest Romans ever got was Galen's theory of microscopic seeds in the air. Even so, humorism and miasma theory were by far and away the consensus among Roman physicians. Neither of which accurately encompassed the origin or transmission of disease. Germ theory as an idea wouldn't become a thing until at least Francesco Redi's experiments in the 16th century.
