Similar to this Stalin died of a stroke because he had given his guards strict orders not to disturb him so when he didn't leave his room for breakfast or his morning meetings they thought it was odd but no one went to see if he was OK. It wasn't until that evening when he had missed all his meetings that one of his aides went in to make sure he was OK and found him unresponsive.
