Leaving aside that the numbers are bullshit, would it really be surprising that a society in which most people lived all their lives in villages of <1000 people were less likely to murder than people who live in gigantic metropolises where they don't even know the names of their neighbors?
Also the type of murders that are most common in modern cities would have been classified as banditry (since they're performed by bandits) in the medieval era, I'm not sure how this affects the murder rate.

I don't know how they took their stats, but it would be reasonable to assume that a guy with his throat slit or skull bashed in had been murdered rather than killed by natural causes. In most places they just tally up homicides and most people point at that when they talk about "murder" rates.
