In Israel how do people get medical care on Saturdays if people can't work because of the Sabbath?
Israel is more "The Jewish People: The State" than it is "The Jewish Religion: The State".
It's fairly secular.

The Pharisees won that debate about 2,000 years ago anon. Saving a life trumps the Sabbath, and they draw very loose lines about where saving a life begins. It's in the Mishnah, but I don't have my copy with me so you'll have to wait a while (until the evening, most likely) if you want an actual citation to a given tractate.
Not to mention there are large segments of non-religious (and non-Jewish) Israelis who don't care about Jewish sabbath observance.

There are gentiles in Israel. Jews don't do their own manual labor to this day.

Jewish law places human life above all else. The classic example is being allowed to drive a car on the Sabbath if your friend is having a heart attack and needs to get to a hospital. Emergency services and the military still operate normally on the Sabbath in Israel.
