"Macaroni" was a very flamboyant and fashionable style in Europe that a poor colonist could not pull off. So by saying "He stuck a feather in his hat and called it macaroni", American colonists were being mocked for their poor fashion sense (and, well, being poor).

Back in 18th century England, a bunch of rich frat boys went around trying to look as douchey and effeminate and "Continental" as possible. This style was called macaroni.
