Not post-op or trans-sex. This is masculinization of female sex characteristics which happens due to genetic enzyme deficiencies in the reactions in the body that make androgens (testosterone, estrogen).

Men and women develop starting with the same structures that then become something else, so the clit for a woman would become the head of the penis for a man under the influence of testosterone.

If there is too much testosterone because of an enzyme deficiency that breaks it down, for example, then the clit will transform into what you see here.
