i really like color palette and the idea of the negative space but it needs a lot of work, the shape of the men makes him look very fat also it seems a little too big compared to the text

keep practicing, don't make the negative space feel like it was put there because of ''le negative space trend''.
Basically both images have to look good without eachother.
Try some more sketches with different images to portray your symbolism.
Like for instance the little cross is there but doesnt add allot.
When u look for better imagery to use try thinking of multiple versions of what u think represents doctors and fish, try to go more simple before reaching for omething like this

hello i think u have a good idea but u need work more in the man shape not only looks fat, he looks shapeless. fish is good. stetoscope may be in the head of the man not arround the graphic.
