Many vision algorithm use a patch-based classification approach with means that a classifier is asked to answer 'Is this patch car, dog or grass?'.
If you have a relatively high 'car' confidence on a patch you can then assume there is a car and proceed with object-specific segmentation/analysis of the car's direction and such.
This is mainly because classifying 'textures' (not actual texture, but any specific region on a 2D image) is well-studied, fast and robust (using LBP and the likes).

Images are repeated in random order to possibly thousands, if not millions of people. A few people clicking on the wrong boxes won't make any difference.
