First is hardware. Most cpus from 2006 onwards have an uncontrollable backdoor that cant be turned off. This has always been exploited at one point and needed to be patched. Then most HDD and SSDs have been shown to be vunerable to firmware attacks. So the two critical components are backdoored.
Second is software, unless you use oss your screwed. Then the matter of security while using the net, especially your footprint is considerable to your privacy.
Thirdly is the government not wanting privacy. Thus laws making isps retain data for years and laws that screw over the 5th amendment for digital stuff, like passwords. In the uk laws inprison you for forgetting a password. In Australia theres not even laws protecting your privacy in the first place.
Theres way more to this but keeping yourself private is difficult unless you make an active effort and use the right software

Yes, but everyone should take some time to pick a security standard and setup communication tools accordingly. That's not gonna happen so lots of people go the easy, less/non secure way.
