This is already happening. To see it:

1) Setup public facing web server
2) Look at logs

You don't need something to be listening on port 80 for very long before you get dozens and dozens of attempts per hour trying to exploit known vulnerabilities in WordPress and MediaWiki and other commonly used content management systems. The already infected systems attacking you continuously don't care if you're not using those, they just attack everything and anything on the off-chance that it works.

Another experiment you should try, setup a ftp server that allow anonymous as username and password and see how long it takes before all kinds of strange stuff is being uploaded.

Yes. If you're a sysadmin and you admit 1000 computers running unpatched exploitable Windows computers then you're at minimum partly to blame.

To their defense, there are some reasons why hospitals got hit. If that half a million dollar x-ray machine only works with Windows XP then that could be problematic. There are a lot of strange equipment in use that require some special Windows version. One solution would be to not connect these things to the network..
