Its a server client thing like email. You could host your own server on your desktop but it would be stupid.

I use it and I'm slowly convincing my friends to use it as well, but it's not there yet.
Just today my friend was sending messages that weren't decrypting on my end.
I use it with friends and I really like it. Haven't had any issues. There are still a few things to implement though particularly with e2e
