someone makes a virus and gives it a PDF file icon and a name like "totally not a virus trust me im a dolphin.pdf.exe". and because Windows is a dumb operating system the default behavior is to hide the ".exe" part of the filename because that's actually what the file is. Generally people who fall for this don't even realize they can open a PDF in a browser or if their browser already does it, they don't realize it's the same program.
