Nova Launcher.

The wallpaper is one of the stock ones from Whicons icon pack

The clock widget is from the base KWGT pack and the weather widget from the Ocea pack; both had some personal tuning.

I used the Lines icon pack for the folders but for the apps in general it's the Pixel icon pack.

Mine's a custom ROM with some features to let me customize a bunch of shit. But if you're running a stock ROM you can root your phone, get Xposed Modules, install Gravity Box and customize a ton of shit, including the icons. You can do it on a custom ROM too but it might break stuff. There might be something easier in the App Store that lets you do stuff like that but I'm not aware of any.
