My Nook HD+ has been running Cyanogenmod for years but haven't really updated since the newer versions have severe performance problem

The company that develops core apps for CyanogenMod (CyanogenOS) is dissolving. Kondik was scared that CyanogenMod and its assets could be sold off so they switched to a new company, LineageOS. But for normal users, it's just a name/website switch.

There are probably some xda threads with the source code, but they took down cyanogenmod.org, so you can't get the official source anymore

They are stopping the development of CyanogenMod by forking it into LineageOS. Basically they'll just continue on as always with another name.
