At one of the hospital meetings one doctor was like 'I can't read this doctor's notes'. It's actually a really big issue to the point that abbreviations are 'officially' forbidden but all the doctors do it anyway because muh timesaving.

Typically all the actual typing is done by medical scribes and often times doctors read out their 'letters' and someone else types out what they say down for them.
