It was a long series of unfortunate circumstances.
- ripping off Xerox but without the proper infrastructure (first based on DOS) or the people
- MS got too big for their hats
- MS hired Ballmer whose attitude was ugly

There was no intrinsic reason it had to be bad. It just turned bad.

Mind you, CDE failed because of equally ugly business but was far less visible. VRML is another case of promising tech attracting caligula class business men.
