I read wikipedias article about the instruction. It helps manipulate data smaller than a 'word'? So smaller than a register in cache?

Cache and registers are different things. Cache is basically a faster ram while registers are what the cpu actually uses to access data (very very very fast, very small)
Tbm offers instructions to do stuff that now has to be done with multiple different instructions (as far as I understand, I'm not that 'fluent' in instruction sets and what they are used for) yes this would make some things slower, and I suppose it's used quite a bit in high performance programming
