IP has been ruled to not be indicative of a person, so they can't fine people unless they're dumb enough to reveal who they are, or without cooperation from the ISP.
They do go after people who pirate, but it's obviously a lot more productive to go after people who offer piracy services - and in the meantime, you don't want to touch their clients so that you can strike them all at once instead of giving them the time to sprout a new service and organize.
