If I walked in and it looked like that I would not touch a thing.
I would plan ahead and look for opportunities to switch the cluster fuck out bit by bit when the opportunity comes. So I would need to plan ahead what I want the whole final product to look like so when whatever random parts need work I know what that part will need to look like.

Trace out patch runs, Velcro together like runs by path, research every patch end to end and confirm.
Start replacing patches with measured runs during maintenance windows.
