Every time i move up in weight my form is super insanely shitty then slowly gets better.
Is this ok? or am I supposed to do more reps with less weight and perfect form until I at least have mediocre form when I increase the weight?

If your form is breaking down that much when you move up, then increase the weight at a slower rate. You did already answer your own question though, my gauge is that if my form on squat for example for 4x8 is really good, then I go up 5lbs.
inb4 don't move up til you feel comfortable and don't have terrible form, you don't want to hurt yourself now.
