It's likely both. Many people live in the suburbs in America and need to drive everywhere because of the convenience over walking. Our culture is basically circled around 'convenience,' specifically in terms of time-management. So people don't move around much and the energy isn't used and people are left with all the fat that could've been burned off. And on top of that, fast-food companies are competing in deals for how cheap they can make food. Kids in college, as well as high school and middle school are gonna go to fast-food places since it's cheap. But the food is cheap only because of it's low quality. As for adults, it seems that they pay incredibly little mind to health and fitness but I don't really know specifically. Likely because of our "treat yourself <3" dynamics in society.

I really can't imagine most suburban dads of the 50's jogging and weightlifting. Maybe they tossed a football with their kids but that's about it. Meanwhile they smoked and drank like a fish and ate steak for breakfast and managed to weigh the same as the average American female today. (166 lbs)

You're leaving out the prevalence of blue collar jobs back in those days, where working for most men actually meant sweating and using your muscles rather than sitting in front of a computer with terrible posture for eight hours a day.

Also those meals were home made, eating out being a treat reserved for extra special occasions.
