Lil notepad and a pen.
People give me weird looks when I pull it out. But whatever, It works.
notepad on my phone, and i take pictures of the last days workout in my journal so i know how many reps and what weight to do. very efficient

Simple Workout Log on Android. Pretty cute, has a little graph to show your progression and allows you to input new excercises and also has a list of ones already in the program and it's pretty easy to use.
