The thing about London is that the gym scene is hugely polarised (at least in central London where I live; it's a bit better as you get towards the outer boroughs).

You either have shitty places (though some of them are 24 hour to be fair) at the £20-30 lower end, or super high tech £120-200+ places on the upper end.

Virgin Active - my gym - is one of the few that actually falls in the middle. And for that I get free towels and pool, jacuzzi, and sauna and steam room access. The facilities are always clean and the equipment good quality.

The only gyms I've tried in downtown proper is Ivan Saliverry MMA, Seattle Athletic Club and Urban-Fit Studios gym up in Capitol Hill.

- Ivan Saliverry: Small actual workout space but usually not overly crowded, moderate selection of weights. You go for the classes, obviously.

- Seattle Athletic Club: obscenely overpriced, machine/class heavy with OK selection of free weights which are always taken up. I had a trial membership when I worked downtown and declined paying $130 a month. That was several years ago and I'm sure it's more now.

- Urban-Fit: for a smaller gym there is almost never a wait at the squat racks and it's usually pretty empty anyway. Pretty good amount of free weights, very few machines. I haven't been downtown for a few years though and IIRC they renovated sometime since I was there last.

I'm in West Seattle now and there's actually two really good gyms for lifters if you don't mind a 15 minute bus ride from downtown. The 24 hour fitness is run by brothers (I think) and they've added a shit ton to their weight room in the last two years, plus cheap membership. The West Seattle Athletic club also got renovated a few years back and has an outstanding weight room, plus good amenities like a swimming pool/sauna/rock climbing wall, etc. It was total shit when I first moved to West Seattle but it's way better now that it's under new ownership.

Most gyms in Italy are actually "sport clubs" associated with CONI or other national-level sport association. CONI requires an health check, so most gyms require an health check to be performed annually. Swimming pools were always an exception.
The annual health check was considered compulsory til few years ago, when it has been clarified that gyms do not need to be associated with CONI (or similar sporting club) and so they do not need to require an annual health check. Nevertheless, most gyms prefer to require this health check for a very simple reason: insurance. If you die or suffer an heatstroke while in the gym the insurance will pay, and the insurance premium is lower if they require an health check for all their members.
Mcfit doesn't give a shit about these details

well, the insurance premium is relevant for smaller gyms; for big gym chains like McFit the difference seems to be negligible, maybe because universal health care is basically free (anyway the certificate for the gym costs usually between 40 € - 70 € ; more expensive variants of the certificate are required if you play a sport at a competitive level) and because there's less the attitude to sue someone else for your own behaviour
requiring a health certificate backfired in some cases: a friend of mine always had heart murmur so he always has been barred from entering a gym no matter what, until few years ago, even if his doctor always recommended him to do some exercise (not cardio obviously) but he simply couldn't do anything but callisthenics
