It was going extremely well until my SI imploded because I tried to #buildmomentum on a foundation of shit.

I took some time off from heavy stuff to get better and then hopped on project momentum because it's free, yes.

When I get fatigued (especially with heavier weights), I tend to lean back and kind of turn it into an ugly incline press instead of doing the proper "head through the window" motion.
What weakness is this? It seems like this reflex moves the weight forward and onto my triceps, so maybe it's weak external rotation in the shoulders?

Because your maximal bench press attempt follows a maximal squat attempt. Then your maximal deadlift attempt follows a maximal squat AND a maximal bench attempt. Your nervous system is going to be taxed from the maximal squat attempt and even more taxed from the maximal bench attempt. If your comp total matches your gym total you aren't trying hard enough.

You also need to be mindful of commands on each lift. Not being used to a command or fucking up with a command, or something you might not notice in the gym will get you red lighted.

In raw, the only thing that will consistently add weight to anybody's total is a belt. A singlet it must wear because it lets the judges see your hip crease during the squat and your butt during the bench press. You <might> get 5-10lbs out of really tight knee sleeves for the squat, but they're mostly worn because they're comfy as fuck and keep your knees stable and warm.

Equipped is another story, experienced lifters can get 100kg+ on their bench or squat.
