That saying doesn't refer to DOMS.
It's about pushing yourself during your workouts. People tend to stop when the lifts get hard and fail to realize that it's only when things get difficult that you're given the opportunity to grow.

You're muscle needs to break down and micro tear, then your muscle rebuilds bigger and better and that shows signs of growth. No pain no gain is a saying that you should take @100% truth. This is what separates the dyels from the people who make it
Intensely defined muscles are largely scar tissue so if that's what you want yes. Strongmen have flabby looking muscles.

No.
But... the saying definitely applies to pushing yourself.
Over a series of training sessions, you need to regularly fly close to the sun (i.e. getting close to overtraining, and then backing off) to get the most out of your workouts in the aggregate.
Training continuously like this is both physically and psychologically demanding.
