If you can pass the physical requirements for being a CATOBAR-launched Navy fighter pilot, you're fit enough to be an astronaut.

The MMU in OP's pic is an APU used only in 3 space missions: STS-41-B by Robert L. Stewart and Bruce McCandless II, STS-41-C by James van Hoften and George Nelson (both missions used MMU #2), and STS-51-A by Joe Allen and Dale Gardner (MMU #3).
