I haven't, but there was a cable machine that was usable at my gym, but didn't lock properly unless you pressed the spring loaded pin in by hand i believe. Management knew, but it would take a week or so to get the maintenance guy around (had to be a corporate maintenance guy)

Got to watch the cable machine drop the pulley at least four different times over the next week
